Get Linked to Medical Care

Our Intake Specialist will complete a quick phone or face-to-face assessment and get you linked to care. No appointment needed. Our Medical Case Management program is the first connection to care for anyone interested in accessing services at Damien Center, which includes:

  • People who are newly diagnosed with HIV and need to get connected to care
  • People who are HIV+ and want help getting back into care
  • People who are HIV+, recently moved to the greater Indianapolis area, and need to get connected to care
  • People who are HIV+ and want to transfer their care to Damien Center
  • Previous patients of Damien Center who want to reactivate services with us

What should I expect at my first appointment?

When you arrive for your first appointment, you are greeted by our friendly reception staff. They may ask for your insurance card and photo ID to verify and protect your identity. You may also be asked to sign a few more forms and pay any co-pays or deductibles due.

Once your check-in is complete, you may wait for about five to ten minutes for our nursing staff to take you to an exam room, collect your vital signs, confirm your medical history, and document any concerns or questions you may have for your medical provider.

Depending on the reason for your visit, your medical provider will come in and assess and diagnose the problem, perform a physical exam if needed, and answer any questions you may have. If you need labs or testing, we have LabCorp on-site and a member of the nursing staff or phlebotomist will collect samples from you.

Before leaving, you may be asked to schedule a follow-up appointment as needed.
