Ready to Start Care?
Existing Patient?
What should I expect at my first appointment?
When you arrive for your first appointment, you are greeted by our friendly reception staff. They may ask for your insurance card and photo ID to verify and protect your identity. You may also be asked to sign a few more forms and pay any co-pays or deductibles due.
Once your check-in is complete, you may wait for about five to ten minutes for our nursing staff to take you to an exam room, collect your vital signs, confirm your medical history, and document any concerns or questions you may have for your medical provider.
Depending on the reason for your visit, your medical provider will come in and assess and diagnose the problem, perform a physical exam if needed, and answer any questions you may have.
Before leaving, you may be asked to schedule a follow-up appointment as needed.
After becoming a patient, you will be able to use our Patient Portal to manage care at Damien Center.
