Our Patient Portal can be used to contact your provider, schedule appointments, view upcoming appointments, or pay your bill! For urgent or same-day medical needs, please call 317.423.0130. Do not use the portal for this purpose. Messages sent during the weekend will not be responded to until regular weekday hours resume.

Damien Center offers an easy and secure way to reach your provider or health care information on the go. By logging into our patient portal from your computer or smartphone, you can access lab results, medical records, bill pay, and more! Our patient portal gives you a direct line to your provider, giving the patient the opportunity to ask any non-urgent medical questions at the click of a button. What is the patient portal/Healow?

Healow gives you access to your health information including labs, medications, and more! Connect with your provider via Healow.

How do I reset my password?

Follow the instructions in the Healow App. You will need your Name, Date of Birth, and access to the email you used to create the account. If you can no longer access this email, or you need further assistance, please contact Damien Cares at 317.423.0130. You can also speak in person to the Clinic receptionist at your next office visit!

Why does my name appear differently on my medical records?

Damien Center and Damien Cares use an Electronic Medical Record (EMR) system that is still progressing to be more inclusive. 

Therefore, you may see your Legal Name displayed instead of your Chosen Name in some locations. We apologize for this and are actively working with our vendor to develop more inclusive patient access. If you’ve recently changed your name on your identity documents, including insurance, we can update this in your medical record if you let us know.

What is the Practice Code for the Healow App?

Our unique practice code is EFGJBD.

Can I get copies of my medical records in the patient portal/Healow?

You are able to view medical records with Healow, however, it is not meant to be a complete medical record. If you require copies of your entire medical record, please fill out a Release of Information (ROI) form for ongoing release or one-time release, depending on your needs. After you have filled out the form, please fax it to 317.423.0608 ATTN: Medical Records

If you need urgent access to your medical records, please call the clinic directly at 317.423.0130 for more immediate assistance.

How can I resume receiving text messages after previously blocking them?

In order to restart text messages send the text message 'START' to this Number '329674' - to resume receiving messages again.

Welcome to the Damien Center Patient Portal!

Our online portal (via eClinicalWorks) gives you secure access to:

• Lab results

• Medication lists

• Messages with your care team

• Visit summaries

• Appointment scheduling

• Billing information

Who Can Use the Portal?

  • Adult patients (18+)
  • You control your own access and can add a trusted proxy.

Parents of children ages 0–13

  • You may have full proxy access unless restricted by a court order.

Adolescents ages 14–17

  • Indiana practice requires additional privacy protections beginning at age 14.
  • Parents lose automatic access at age 14.
  • Teens choose whether a parent/guardian can have full or limited proxy access.
  • Sensitive services are never available to proxies.

What Parents Can See for Teens (12–17)

Parents may see:

  • Appointments
  • Immunizations
  • General administrative information

Parents cannot see (unless authorized):

  • STI testing/treatment
  • HIV‑related visit narratives
  • Mental health notes
  • Substance‑use treatment
  • Reproductive health services
  • Any confidential note or test result

Teens must sign a Proxy Authorization Form if they want to allow additional shared access.

How to Sign Up

  • Provide your email during registration.
  • You will receive an email invitation from eClinicalWorks.
  • Create your login.
  • Download the healow app for easy access on the go.

Your Responsibilities

  • Keep your username and password secure.
  • Do not share login information.
  • Use the portal for non‑urgent messages only. Call 911 for emergencies.
  • Notify us if your email or phone number changes.
